Athletic Registration & Forms for 2019-2020 (FamilyID)

The HGCS Athletic Department is excited to introduce our new process for athletic registration and form submission for the 2019-2020 school year!

We have partnered with FamilyID, a secure online platform for submitting all necessary information for sports participation. Families will be able to create an account, register middle school and high school students for sports each season, and digitally sign athletic forms. Sports physicals, which must be updated and signed by a doctor every 395 days, may be uploaded directly to your FamilyID account.

Each family will only need to create one FamilyID account. Multiple children can be registered for multiples sports seasons under one family account.

Hickory Grove's FamilyID site is now open and accepting registration for fall sports. Tryouts will be held the week of July 29th, and specific days and times for each sport can be found on the FamilyID page. Each student-athlete's online registration must be complete and sports physical must be uploaded (or submitted in person to the Athletic Office) before tryouts begin. You can begin your athletic registration now by following the instructions below:

REGISTRATION PROCESS:
A parent/guardian should register by clicking on this link:
Follow these steps:
  1. To find your program, click on the link provided above and select the registration form "Hickory Grove Christian School Athletic Registration Fall 2019" under the word Programs.

  3. Create your secure FamilyID account by entering the account owner First and Last names (parent/guardian), E-mail address and password. Select I Agree to the FamilyID Terms of Service. Click Create Account.
  4. You will receive an email with a link to activate your new account. (If you don’t see the email, check your E-mail filters (spam, junk, etc.).
  5. Click on the link in your activation E-mail, which will log you in to FamilyID.com
  6. Once in the registration form, complete the information requested. All fields with a red* are required to have an answer.
  7. Click the Continue button when your form is complete.
  8. Review your registration summary.
The link to access Hickory Grove's FamilyID site is also available under "Athletics" on the school website. The NCISAA sports physical form is attached to this email and is also available as a PDF link on the FamilyID site. The sports physical form will need to be printed and signed by a doctor before it is uploaded to your FamilyID account or submitted in person to the Athletic Office.

We hope that our new online registration will make the process of submitting athletic forms quicker and easier for our athletes, parents, and coaches. If you have any questions about how to access and complete your registration, please contact the Athletic Office at (704) 531-4038. You may also contact FamilyID's technical support team at (888) 800-5583.
